On-Track to Graduation Plan

The purpose of this letter is to let you know the exact risk factors that could possibly prevent your child from graduating. Just because you received a letter, does not mean your child won’t graduate. It simply means we want to work with you and your child on a plan of supports so we can get your child back on track.

When a student is identified as needing a personalized graduation plan (late July of every year), the student will be included in our On-Track to Graduation Plan throughout the upcoming school year. Our goal is to provide enough supports to students during the year so that they no longer need to be included in the plan by the end of the school year.

Students with disabilities can still be identified as needing a personalized graduation plan. The factors that contributed to being identified as at risk may be disability related. The IEP can serve as a tool to address disability related needs and identify interventions and supports required to make progress towards graduation. This will assure that school staff are all working together to address your child’s needs.

While graduation seems a long way off, what your child is doing now will make a difference as they get closer to graduation. Grades and attendance are the result of good habits that your child is developing now in middle school. Students who struggle with grades and attendance in middle school often struggle in high school as well. We want to provide supports early so your student develops the habits and skills needed to succeed in high school, graduate on time and is ready for whatever they want to do after high school.

It is possible that a student could have two risk “factors," and still be on track for graduation. If your student has the credits they need to graduate on time, despite having some areas of concern, we would want to celebrate your student’s success and provide supports to ensure that they continue to stay on track for graduation.

Reach out to your student’s school counselor or your school’s point of contact listed on this website- they will connect you to resources or other student services staff, such as social workers and school psychologists.  If you don’t know your student’s counselor or don’t feel comfortable talking to them, you can also talk to one of your student’s teachers.

That is OK. We are here to help you. The best place to start is with your child’s home school.  Each school has a staff member that can provide supports for you and your child and answer questions you may have. They can also connect you with resources and other individuals to support you.
