Food Allergies & Special Dietary Needs
If your student has a food allergy, intolerance or requires any special dietary accommodations, please follow these steps so we can provide them a breakfast and lunch meal specific to their medical needs.
1. Complete the Form: Print off the "Diet Modification Form" below and have it filled out and signed by a licensed medical practitioner (a healthcare provider who can prescribe medication).
2. Submit to School Nurse: Once completed, give the form to your student’s School Nurse or have the doctor's clinic fax it into the Nurse's Office.
3. Contact with Food & Nutrition Department: The School Nurse will then contact the Food & Nutrition Department.
4. Consultation with Registered Dietitian: A Registered Dietitian from MMSD District may contact you or the School Nurse for clarification or to discuss the dietary accommodations.
5. Allow Processing Time: Please allow 5 days for the accommodation to begin after the form has been submitted to the School Nurse.
This process ensures that the necessary parties are informed and that proper accommodations can be made for the student's dietary needs.

Please note that our District-wide menus do not contain peanuts, tree nuts, fish, or shellfish. While our menu items are free from those ingredients, some of our products are made in a facility that may contain those allergens.
