Life Insurance
Basic Coverage (as of July 1, 2018)
Benefits eligible employees are automatically enrolled in term life insurance and accidental death and dismemberment insurance equal to one times your annualized earnings rounded up to the next $1,000. For example, if your earnings are $18,200, coverage is $19,000. The District pays 100% of the premium. Coverage will increase as your earnings increase and premiums increase with your age. Also included in the basic coverage is an accelerated benefit, up to 50% of coverage and conversion options.
Voluntary Coverage (as of January 1, 2019)
In addition to the Basic Coverage, you can elect Voluntary Coverage for yourself, spouse and eligible dependents. This plan is available to you if you elect the Basic plan.
Employee Coverage Coverage is available in increments of $10,000, up to $300,000 (or 5x your salary, whichever is less). Coverage ends when employment ends, or if a retiree, at retirement or age 65, whichever occurs later. Portability and conversion is available if you leave MMSD.
Spouse Coverage Coverage is available in increments of $5,000, up to 50% of your Voluntary Coverage for yourself.
Dependent Coverage Coverage is available at $5,000 or $10,000. If you enroll in this coverage, you pay just one monthly fee and it will coverage your spouse and/or dependent(s), there is not a per person monthly premium. Premiums are $0.90 or $1.80 depending on the coverage tier.
Enrollment in Voluntary Coverage
To enroll for coverage, you must complete and submit a group life insurance enrollment form within 30 days of becoming eligible for coverage. If the enrollment form is received after the 30 day enrollment period, you will be required to undergo insurance underwriting for the application to be processed (coverage may be approved or denied base on the underwriting).
