Reasonable Accommodations

Under the federal Americans with Disabilities Act (ADA), employees with a disability may qualify for reasonable accommodations to perform the essential functions of their job.  Accommodations are considered “reasonable” if they do not create an undue hardship on the employer. 

To apply for a reasonable accommodation, an employee must work with their health care provider to identify what accommodations are recommended to perform the employee’s essential job functions.  The employee must also engage in an interactive process with a Human Resources representative to identify options and determine if a requested accommodation is reasonable. After working with the employee, a human resources representative will work with an employee’s supervisor to identify an employee’s essential job functions and determine what accommodations are reasonable.

Requesting an ADA Reasonable Accommodation

To request an ADA Reasonable Accommodation, employees should discuss their needs with their health care provider and the employee and the provider should complete the Request for Accommodation form.  Completed forms should be submitted to accommodations@madison.k12.wi.us.

A note to Supervisors

If an employee asks for an adjustment to their position due to a medical condition, please refer the employee to Human Resources at accommodations@madison.k12.wi.us. The employee does not need to use terms “ADA” or “Reasonable Accommodation” to be referred for an accommodation.
