MMSD will be continuing the opportunity for employees enrolled in MMSD’s health insurance to take part in a wellness assessment. The wellness assessment includes a biometric screening and a health risk assessment. Those not enrolled in MMSD's health insurance should not complete the MMSD wellness assessment, but can receive similar information at an annual physical.

Welcome back to the 17-18 school year!

MMSD will be continuing the opportunity for employees enrolled in MMSD’s health insurance to take part in a wellness assessment. The wellness assessment includes a biometric screening and a health risk assessment.

There are two important reasons you will want to consider participating in the wellness assessment this fall.

  1. Your Health insurance contributions are based on your participation (or the lack of participation) in the wellness assessment.  If you choose to not participate in this year’s wellness assessment, you will pay a higher health insurance contribution premium beginning on your January, 2018 coverage (as you will be paying the non-participant rate).
  2. It's important to follow your health year-to-year.  By participating annually, you are able to track your results with the wellness assessment results.  Additionally, it may highlight any changes to your health, which you will want to discuss with your physician.

How To Participate:

The wellness assessment is two parts – a biometric screening and an online health risk assessment that is completed by you (your spouse, domestic partner and/or children do not participate).  The wellness assessment closes on November 30, 2017, with both* steps needing to be completed by then.

Not completing both steps will result in a higher health insurance premium for your 2018 health insurance coverage.

Step 1. Complete the Biometric Screening

1.      Sign up for a Biometric Screening Appointment at www.myinterrahealth.com. To sign into your account, please use the following information.

a)      Participant ID: enter your “b” number (just the numbers)

b)      Password: If you are a new employee, enter in “wellness” as a default. If you have forgotten your past password, please click Forgot Password.

c)      Company ID: enter in MMSD

2.      Attend your Interra biometric screening appointment

OR

3.    Complete the biometric screening with your physician. Your physician will need to complete the Physicians Results Form and it should be returned to Interra. Please note, this option is available to you if you do not complete the Interra biometric screening.

Step 2.  Complete the Online Health Risk Assessment through your health insurance carrier (GHC)

GHC Directions

1.      Go to GHC website www.ghcscw.com

2.      Log in to your GHCMyChart

a)      If you do not have a GHCMyChart account, click on “Register & Activate,” to create one

3.      Go to Resources and click on “WebMD Portal”

4.      Proceed To WebMD. A landing page will open on GHCMyChart; click on the red “PROCEED TO WEBMD” box at the bottom of the page

5.      WebMD website will open

a)      NEW USERS will need to register for the site

b)      RETURNING USERS will enter automatically

6.      Take the WebMD Health Quotient (health risk assessment; questionnaire; survey)

a)      NEW USERS: It will open automatically after you register for the site. To take it, click on “Take It Now”

b)      RETURNING USERS: It will open to the WebMD Home Page. Click on the HEALTHY LIVING Tab at the top of the page, and then click on HEALTH QUOTIENT

 If you HAVE NOT taken it before, click “Take It Now”

                                                                       ii.

 If you HAVE taken it; and need to RE-TAKE it, click “UPDATE SCORE” next to your SCORE (0-100)

                                                                      iii.

It will say “Update Your Health Score,” click “OK”

7.      Go through each page, and click “Save & Continue”

8.      When all questions are updated, click “Save & Finalize”

9.      Rank your readiness to change, click “Save & Continue”

10.   Rank your evaluation & time, click “Finish”

11.   See your score & scroll down to read your reports

12.   Log Out (upper right hand corner--click on your Name then down to Log Out); and then Log Out of your GHCMyChart

13.   Contact your insurance carrier to confirm the completion of the assessment

DEAN DIRECTIONS

1.      Go to Dean Living Healthy website

a)      NEW USERS will need to register for the site

b)      RETURNING USERS will need to enter using their Living Healthy Username and Password.  The Login and Password can be reset by clicking “Forgot Your Username or Password”

2.      Take the WebMD Health Assessment (health risk assessment; questionnaire; survey)

a)      NEW USERS: It will open automatically after you register for the site. To take it, click on “Take It Now”

b)      RETURNING USERS: It will open to the WebMD Home Page. Click on the HEALTH ASSESSMENT Tab at the top of the page, and then click on UPDATE SCORE

3.      Go through each page, and click “Save & Continue”

4.      When all questions are updated, click “Save & Finalize”

5.      Rank your readiness to change, click “Save & Continue”

6.      Rank your evaluation & time, click “Finish”

7.      See your score & scroll down to read your reports

8.      Log Out (upper right hand corner--click on your Name then down to Log Out)

9.      Contact your insurance carrier to confirm the completion of the assessment

Note: Even if you're pregnant or need an accommodation, you will want to participate.  You can fill out a waiver for part of the assessment because of a pregnancy or serious medical condition (which can be found on Interra's website after your log into your account) - but there are other parts you will need to participate in to be eligible for the wellness premium credit.

Effective January 1, 2018, your health insurance will be based on your participation in the wellness assessment (the biometric screening and health risk assessment). See premiums here.
