Guidelines for Medication at School

If you would like your child to receive any medication (even over-the-counter medication) at school, you will need to provide the school nurse with the following items:

  1.  A parent/guardian consent form. This is your permission for school staff to administer medication to your child.

    Consent Form (Hmong)
  2.  A written medical order from your child’s Health Care Provider.
  3.  Prescription medication must come packaged with the prescription label that contains the child's name, name of the medication and dosage, time to be given and practitioner's name.
  4.  Over-the-counter medication must come to school in the original manufacturer packaging.
