At some point, the inevitable happens and you’re injured while at work. Hopefully, the injury is small – a paper cut or scratch, but other times it may be more severe. In every situation, it’s important to remember to let the HR Department know that you were injured. There are a few steps in the notification process that you will want to follow:
1. Complete the First Report of Injury immediately following the injury. You should return this form to the HR Department as soon as possible, but no more than 3 days after the injury.
The First Report of Injury can be returned by emailing it to email@example.com or faxing to 608-204-0346.

2. If you would like to speak to a Registered Nurse confidentially about your injury, at no cost, you can contact one at 1-855-650-6580.
3. If you need to seek medical treatment, please do so.
You can be seen by any certified physician. Please be aware, you cannot seek treatment from a Nurse Practitioner.
Take the First Report of Injury Packet with you to your appointment and have your provider complete the Work Status Report/Medical Service Form.
If you have any work restrictions, please notify the HR Department. We will work with you to get you back to work as soon as possible.
4. If you miss work due to your injury, contact the HR department at 608-663-1745.
